The following form will be sent via Email to the Health Department.  Please DO NOT include private, personal or confidential data as information will be transmitted via a non-secure messaging system and may be intercepted by a party that you do not intend. Emails received may also be forwarded to other County departments for an answer. 

You may expect a response from the Health Department within two working days.
