The PATH program serves individuals with serious mental illness, as well as individuals with co-occurring substance abuse disorders who are homeless. PATH, by nature of this selective focus, draws attention to the most vulnerable persons within the homeless population by providing linkage to treatment and other resources and support to obtain and maintain stable housing.

PATH is a federal grant program, created in 1991 under the McKinney Act and administered by the Substance Abuse and Mental Health Services Administration (SAMHSA).
