ADRC Youth Transition Services
The Aging and Disability Resource Center of Rock County assists with the transition process of disabled youth to adulthood starting at the age of 17.5 years.  The ADRC can provide information to the youth/student and family on transition planning, publicly funded long-term care, integrated employment and helping youth achieve personal goals that they may have after school.
