Overview

Our community partnerships are rooted in the belief that we are stronger together. Health is shaped by decisions across all sectors, making collaboration essential to improving population health and removing barriers to well-being.

We work alongside a diverse network of partners, including community organizations, healthcare systems, schools, businesses, local municipalities, libraries, and government agencies to address complex public health challenges that no single organization can solve alone.

In addition, we are proud to support and facilitate the Health Equity Alliance of Rock County (HEAR), Nutrition Collaborative of Rock County, Thrive by 5, and three fatality review teams. These coalitions bring together local organizations, businesses, non-profits, civic groups, and residents to advance shared goals.
