Every three years, Rock County Public Health completes a community health assessment (CHA) and develops a community health improvement plan (CHIP), as required by state law.
The Community Health Assessment (CHA) provides a snapshot of the community’s health through comprehensive data collection and analysis. The data helps identify opportunities to improve community health. These strategies develop the Community Health Improvement Plan (CHIP). The Community Health Improvement Plan (CHIP) is a long-term, systematic approach to improving community health. The Health Equity Alliance of Rock County (HEAR) facilitates the development of the CHA and the implementation of CHIP strategies on a 3-year cycle. The current CHIP priorities are housing and mental health.
Many organizations and community groups utilize CHA data to guide their program and services. For this reason, the CHA-CHIP has been integrated into the Healthy Rock website to serve as a data resource for the community.
