• Temporary Food Stands
• Mobile Food Trucks or Carts

• Hotels, Motels, Bed & Breakfasts
• Tourist Rooming Houses
• Tattoo or Body Piercing Shops
• Public Swimming Pools, Whirlpools, and Water Attractions
• Public Beaches
• Manufactured Home Communities

*Use for temporary food stands, campgrounds, or body art stations.    Please note that you do not need to fill out an application if you currently hold a temporary permit.  A renewal for the next licensing year, which starts on July 1st, will be sent to you in May.

• Please mail or email completed application.
• Include other applicable information such as menus, floor plans, and certifications.
• Contact the local municipality for building code, zoning, fire, and other requirements.
• DO NOT submit fees until amounts can be verified by our department
